On your first visit, and follow up visits, I encourage you to be specific about what results you hope to achieve with massage.

Is it pain relief?        sports injury recovery?          stress reduction?        relaxation?      all of the above?    

The information you provide will ensure that the type of massage you receive is specifically tailored to you.

Don’t worry, you don’t need to know if you need Swedish, deep tissue or Structural Relief Therapy (SRT).  The information you give me, will help me make the best decision about what type of massage you need to feel your best.

Massage Services

30-Minute Therapeutic Massage – $47
This treatment is perfect if you have one area that is painful or sore. It is also effective for sports or occupational injuries.

  • Multiple 30 Minute sessions discount
    • Purchase five (or more) 30 Minute sessions – 10% off
    • Discounted 30 Minute sessions cannot be combined to be used as a 60-Minute session.
      • To receive multiple session discounts, the treatments must be booked and paid for on the first session.
      • Unless there is a special offer in place, multiple sessions are to be used by the same person.
  • Multiple 30-Minute sessions discount – ONLY available in the month of DECEMBER
    • Purchase five (or more) 30 Minute sessions – 15% off
      • Discounted 30-Minute sessions cannot be combined to be used as a 60-Minute session.
      • During the month of December, you are allowed to purchase these sessions for yourself, or give them as gifts to others.
      • Please call the office regarding gift packs of 5(+).  If you purchase them online using eGift, you will NOT be able to distribute them to more than ONE person.

45-Minute Therapeutic Massage – $68
Is 30 minutes too short…and 60 minutes too long? This this is the perfect treatment for you!

60-Minute Therapeutic Massage – $85
A 60 Minute massage is beneficial for those who:

  • Need to relax
  • Have some areas of pain, but would like a complete massage
  • Have two or more areas of concern that need specific work

90-Minute Therapeutic Massage – $125
This is the ultimate treatment. For those clients who feel that 60 minutes is just not long enough!

Student 30-Minute Therapeutic Massage – $45
This session is to be used by either high school or college students.  High school students must be accompanied by a parent or guardian per State Regulation 5.02, #8 & #9

Hot Stone Massage – $120.00 (75 Minutes)
Hot Stone Massage is known for its deep, relaxing benefits due to the heat of the stones. The smooth basalt stones are placed on specific points of the body. This helps to melt away muscle tension and stress. The stones also used to massage the arms, legs, neck, and back.

Active Isolated Stretch (AIS) the Aaron Mattes method

30-Minutes – $47 / 60-Minutes – $85 / 90-Minutes – $125
Active Isolated Stretch is a different method of stretching.  Clients should wear or bring loose fitting clothing.  It is a safe and effective way to stretch, and it can help relieve the following:

  • Neck pain
  • Low back pain
  • Limited range of motion
  • Poor posture
  • Decreased flexibility

The benefits include:

  • Increased circulation
  • Improved elasticity
  • Increased muscle tone
  • Increased range of motion
  • Reduced muscle spasms
  • Reduced muscle and tendon injuries

Office & Financial Policies

Scheduled appointments
All treatments are by appointment only. Walk-ins are not accepted at this time. All appointments begin and end on time. Every time. Guaranteed*. (*For each minute the therapist is late, the client will receive $1 off the price). All appointments are for the full time allotted.

All clients under the age of 18 will be required to have a parent or guardian present during the entire treatment as regulated by state law, 5.02 (8), 5.02 (9).

Late Policy
If the client is late, the appointment will be shortened and end according to the original start time of the appointment, and the client will be charged the full treatment fee.

Missed Appointments
Your treatment time is reserved specifically for you. Please provide a minimum of 24-hours notice to cancel an appointment. Clients who do not call within 24-hours will be charged the full appointment fee.  If you are ill or have a family emergency, please call as soon as you are able.  Voice mail is accessible 24 hours/day. You may call and leave a message at any time.  Emergency cancellations and fees will be assessed on a case-by-case basis.

Payment
The client is responsible for payment of all fees. Payment is expected at the time services are rendered. Deb Levesque Massage Therapy can provide a receipt of payment for clients who need to submit proof of treatment and payment to their insurance carrier or their flexible medical spending account provider.

The office accepts: cash, personal checks, debit cards, VISA, MasterCard, American Express and the Discover Card. Checks are made payable to: Deb Levesque Massage. A $35 fee will be charged for any returned checks. Clients may pay for multiple treatments in advance if they wish to.

Gift Certificates
Gift certificates can be purchased for a specific length of treatment time (ex. 30 minutes, 60 minutes, etc.) or for any dollar amount. The policy for no-show or cancellations of gift certificates is, if the appointment is canceled with less than 24-hour notice, the gift certificate will be considered payment for the missed treatment and will be voided.

Forms
All clients will be asked to fill out a medical history form. You may print this form and fill it out prior to your first treatment. By signing this form, you give permission to Deb Levesque Massage Therapy to send cards, newsletters or other clinic documents to the mailing address and email address listed on the medical history form. The client may remove their name from such mailing lists at any time. Deb Levesque Massage Therapy keeps all contact information confidential.
