Treating clients right is something Deb takes seriously. Upon making the first appointment she will need to know what results you hope to achieve with massage, i.e., pain relief, sports injury recovery, relaxation, etc. This will ensure that the length of time and type of massage you receive is specifically tailored to you.

Office & Financial Policies
Scheduled appointments
All treatments are by appointment only. Walk-ins are not accepted at this time.
All appointments begin and end on time. Every time. Guaranteed*. (*For each minute the therapist is late, the client will receive $1 off the price). All appointments are for the full time allotted.
All clients under the age of 18 will be required to have a parent or guardian present during the entire treatment as regulated by state law, 5.02 (8), 5.02 (9).
Late Policy
If the patient is late and treated, the appointment will be shortened and end according to the original start time of the appointment. Late patients will be charged the full treatment fee.
Missed Appointments
Your treatment time is reserved specifically for you. Please provide a minimum of 24 hours notice to cancel an appointment.
Last minute cancellations or no-shows result in loss of revenue to the practitioner. Clients who do not call within 24 hours will be charged the full appointment fee.
Voice mail is accessible 24 hours/day. You may call and leave a message at any time.
Payment
The client is responsible for payment of all fees. Payment is expected at the time services are rendered.
Deb Levesque Massage Therapy can provide a receipt of payment for clients who need to submit proof of treatment and payment to their insurance carrier or their flexible medical spending account provider.
The office accepts: cash, personal checks, debit cards, VISA, MasterCard, and the Discover Card. Checks are made payable to: Deb Levesque Massage. A $25 fee will be charged for any returned checks. Clients may pay for multiple treatments in advance if they wish to.
Gift Certificates
Gift certificates can be purchased for a specific length of treatment time (ex. ½ hour, 1 hour, etc.) or for a specific dollar amount. The policy for no-show or cancellations for gift certificates is, if the appointment is cancelled with less than 24 hours notice, the gift certificate will be considered payment for the missed treatment and voided.
Client Rewards
As a thank you for being a client you will receive a birthday card with a $5.00 off coupon to be used on your next treatment.
Clients that refer others to this office will also be rewarded with a $5.00 off coupon when the new client uses your name as a reference. Word of mouth is the best marketing, and I am happy to reward clients that help this office grow!
Forms
All clients will be asked to fill out a medical history form. You may print this form and fill it out prior to your first treatment. By signing this form, you give permission to Deb Levesque Massage Therapy to send cards, newsletters or other clinic documents to the mailing address and email address listed on the medical history form. I understand that I can remove myself from such mailing lists and that Deb Levesque Massage Therapy will keep my contact information confidential.
